Confirm your identity before accessing a medical request form – automatically with photo ID and a selfie, or through our accessible route.
Request and GP review
Complete the secure questionnaire and consent, then pay the displayed fee. A GP reviews each request as promptly as possible; more evidence or a consultation may be needed.
Clinical outcome
If approved, your signed letter is provided as a PDF. We will email instructions for receiving it securely. If no letter is issued, you receive a full refund.
